World War I History Commission Questionnaires

Military Service Record and Nurses' Questionnaires submitted by WWI military and civilian personnel. Also includes content submitted by counties and cities such as draft board records, solider biographies, and military unit histories.

Format of Questionnaires

Each Military Service Record questionnaire has four pages:

  • The first page records information about personal background, including full name; date and place of birth; name of mother and father (and their places of birth); race; religious affiliation; names of spouse and children; fraternal orders and college fraternities; education; occupation prior to entering the service; name of employer; residence before and after time in service; and present home address.
  • The second and third pages record details of the individual's war record, including date of induction, rank, military branch, and ID number; military company, regiment, and division; where trained or stationed before going to Europe; port of embarkation and debarkation; experiences in action; citations for and details of distinguished services; details of any injuries; discharge; and occupation after the war.
  • The fourth page contains several questions designed to elicit information about how the war affected the individual. The questions deal with attitude toward military service; the effect of disabilities on occupation and employment; the mental and physical effects of United States camp experiences and of overseas experiences; how the experience affected religious beliefs; and the effect of all of these experiences as contrasted with the individual's state of mind before the war.

Each Nurses' Questionnaire has two pages:

  • The first page records information about personal background, including full name, place and date of birth, home address, and address before entry into the Service; race; religious affiliation; marital status; education and professional affiliations; service/enlistment date and place; camp or hospital where stationed before going to Europe; and personal experience in a camp or hospital before going abroad.
  • The second page records embarkation date and location, route and means of transport to arrive at the assigned post, narrative of experience with the Service, handicaps, deficiencies, supplies, results achieved, additional data, and discharge date and location. 

Note: Some nurses and civilians (e.g., interpreters) completed the Military Service Record questionnaire.
